This sample db demonstrates creation (or deletion) of an array of four command 
buttons at run time.

Controls can be created or deleted only when the form is in design mode in design 
mode.

Use of temporary auxiliary forms makes it look as if the creation (or deletion) of 
controls is taking place directly on clicking of appropriate command buttons on the 
form itself.
